  • Providers must include essential information: names, specialties, addresses, and telephone numbers.
  • Cultural and linguistic capabilities (including languages offered) must be listed.
  • Availability for new Medicaid and CHIP patients should be indicated.

CMS Interoperability and Patient Access Final Rule:
